Release gitlab issue template does not work

The release template for issues has simply not worked for at least the last year, I think longer. When I select it, the page gets stuck loading the template, and never actually does anything. I've had to manually copy the issue template from the repo and paste into an issue to make release issues.

I am assuming we have an error somewhere in our template, but it is also possible that there was either an update to gitlab that is not backwards compatible or that this is a gitlab issue (though I think this last is the least likely).
