Distance marginalisation lookup doesn't store phase marginalisation

The distance marginalisation lookup table depends on whether phase marginalisation was assumed when constructing it.

We should add phase marginalisation as an extra flag to the checks done when loading the lookup table.

If you load a phase marignalised table without using phase marginalisation (or vice versa) things go very wrong.
