This is a squash of several commits that introduces
a new struct of waveform flags into the LALSimulation
ChooseTD/FDWaveform functions. It also adds a number of
functions to manipulate the struct and changes all calls
to ChooseTD/FDWaveform.

ChooseWaveform functions and control special behavior of

 * #define LAL_USE_OLD_COMPLEX_STRUCTS in .c files which still use old
   LAL complex structs. (Definition must appear before any LAL header
   #include.) Code in these files can be ported to using C99 complex
   datatypes individually, after which the #define should be removed.
