This reverts commit a1a082f7.

Fixes #231.

The function currently assumes that only detectors with a valid
chi^2 field contribute to the ranking of an event. This is not
true for PyCBC Live in O3, which can have empty chi^2 values even
though *all* detectors always contribute to the ranking.

This patch fixes the get_instruments_in_ranking_statistic()
function to always return all detectors for events from
PyCBC Live.

Included posterior sample file name in the log message.

templates/online_pe.jinja2: use nodes dedicated to online PE also for playground events so that the test runs do not get stuck due to the lack of resources.

This reverts commit 5701a2ac.

Fixes #243.

Event complateness i.e., presence of p_astro, skymap and em_bright
labels on CBC and skymap for Burst events are taken into account
into the ``should_publish`` criteria. Also the preferred event of the
superevent will be frozen once the ``EM_Selected`` label is applied
to a superevent.

.bashrc, tasks/inference.py and template/online_pe.jinja2: use mpich as the MPI runtime for parameter estimation.

It's my theory that it is dominating the run time.
