Add favicon

Geoffrey Mo requested to merge geoffrey.mo/gracedb-server:favicon into master

I know nothing about web dev but I often have many tabs open and it would be nice to be identify which ones are the GraceDB ones.

I probably missed something (or many things) in this MR but would be happy to fix what's needed to get this to work.
