remove preliminary_alert_timeout pathway entirely

The preliminary_alert_timeout value is set to zero for about two years now. We have talked about getting rid of it entirely. Evidence from several MDC runs have shown no issue with this timeout being absent. This patch remove the pathway and config variable.
