Add form to create sky maps with disabled detectors
Fixes #754.
Note, I don't know how to test this because I am lacking information about GWCelery deployments and configurations (see #756 (closed)).
Maintenance will be performed on git.ligo.org, containers.ligo.org, and docs.ligo.org on Tuesday 30 April 2024 starting at approximately 9am PDT.
