Remove cacheentry
See !27 (merged) (@duncanmmacleod @kipp.cannon), here, as discussed, the CacheEntry class is completely removed from glue. As 2 other modules use CacheEntry within glue there are also changes there to point to lal.utils.CacheEntry.
