Reverted signal model changes done by Prathamesh to get branch back in approximate state of prelim O3a/O3b runs

- the header files in the lalinspiral are a total mess, and as I clean them
  up I'm discovering other codes that have been omitting some headers
  because they were being pulled in from lalinspiral files.  this probably
  won't be the last time I have to adjust things before the dust settles.

gstlal_inspiral_create_prior_diststats: fix issue where bandwidths were only considered for a single template per bank

inspiral_pipe.py: fixed bug that prevented rerank dags from working generally. Essentially reverts part of Codys last push

already completed the calc rank pdf and marginalization steps

inspiral_lr.py: Fixed handling of dtdphi = None. This is/was fixed on master, O3b_online, and O3b_early_warning; I do not know how it was missed here.

inspiral.py: change the naming convention for gracedb files to remove redundent instrument information

gstlal_inspiral_plotsummary: modify event table columns to consolidate template properties, add UTC time column

inspiral_lr.py: Needed to add dtdphi reference to the splice class method in OnlineFrankensteinLnSignalDensity. Should have been included in original dtdphi push, but it slipped under my radar until skymap dag testing.

gstlal_inspiral: make sure anything with a reference to pipeline is freed before setting NULL - NOTE check in on bottle route stuff. Still not sure it is freeing everything it should be in each loop
