- some previous patch accidentally included a change of the LR threshold to 5.0, reduce it to 0.

- allow number of samples to be disabled so that program only plots a previous chain
- also, periodically flush the HDF5 file during sampling

- attach as attribute to custom dict subclass instead of including it in
  the param dictionary and needing to provide overrides of methods we
  don't really need to override.  performance improvement

- saw all the burst codes doing this and thought we'd forgotten to do it
  here, but we don't need this and do expect 1-instrument events to work

- also fix min_ratio to 0.1
- and move some arithmetic out of the loop

- how handled by middleware in pylal.  faster

- also allow program to be restarted from a saved chain
