implement 2D-grouping in svd bank using orthogonalized 0/1PN order phase terms

This patch is to enable 2D-grouping in svd bank using orthogonalized 0/1PN order phase terms.
Though this patch "works", there will be some subtlety to be optimized and some docs/ command line options need to be cleaned up. After we make sure this works best compared to other grouping scheme, I will finalize this patch.
