update SVD plotting script and lvc_rates injection stuff

• gstlal_inspiral_plot_svd_bank had to be updated. This MR removes the CGI commands and changes the output file name such that it is named by the template id.
• The lvc rates and pop injection generation code pushed to gstlal-ugly needed some clean up. This MR adds the names of the programs and modules in their respective Makefile.am, and fixes the import commands in the programs.
