Improvements to the README and CONTRIBUTING guide

Links are added cross-referencing the readme and contributing guide. Improvements are added so that a new developer can follow the fork-pull workflow without getting into trouble. The updates are not the only way, but they work and avoid developers from guessing which approach to take.
