Add tool to combine sky maps from different observatories

Tito Dal Canton requested to merge tito-canton/ligo.skymap:ligo_skymap_combine into master

As requested in #2 (closed). This script tries to be as generic as possible while being immediately usable to combine LIGO/Virgo and Fermi/GBM skymaps. I only did a few quick tests with skymaps related to GW170817 / GRB 170817A.
