General cleanup. Removed unused files, updated changelogs. Minor doc update.

Added some description to NDS::channel_data_type_conversion. Removed many shell files that were left over from the old autotools tests. Removed an unused .cc file (nds.cc), and an unused and outdated doxygen configuration file. Updated changelogs.
